you will also be provided with information about how the blood samples should be sent.
If you are the patient, please take the lab request form with you to your treating physician.
Patients can also fill out the form themselves to be sent in with a blood sample.
If you are a citizen of one of the following countries, please request the maintrac® lab form and the blood sample set from
the local distributor.
